The Foundation will address the needs and provide support for individuals who want to be empowered to change and secure independence.
Goals and Objectives: (Will be determined by the client's specific need request)
Each participant will provide documentation to verify a need for services.
- With 85% compliance, each participant will demonstrate a need by completing a referral form, providing proof of need, and providing a history to show the need for the service.
- Each participant with a 100% response will complete the evaluation form and give evidence that the service has successfully been accomplished.
- Each participant will give permission for their personal information to be included in the Foundation's database and will agree to participate in the promotional efforts of the Foundation. This information will be secured so as to protect the privacy of the client.
